Since my world history is not that good, and its even worse when it comes to america, I'm just gonna go with the stab in the dark method. Did it happen near a railway track ? How many old ladies are there we talking about here ? If Tom did die, what would he die of ?

Was Tom the only one who was wanted by the killers, or who would have been killed by the killers? Was he marked out by the killers individually? That is, did the killers have the task of killing him, specifically, in mind? Maybe it was dirty, and the killers in the carriage, or the horses, were terribly blinded by the sight of old ladies' ankles and there was shrieking involved, and Tom assumed that it was about him; meanwhile, in all the commotion of wild horses, the killers accidentally kill themselves (because guns r bad) and thus Tom is safe.
