Were who the ladies were of any significance, or could they have been any old ladies?

Was Tom asleep in his room. The old ladies were crossing the road very slowly. A horse crriage was waiting for them to cross. The driver of the carriage shouted to the old ladies to "make haste" so he can get on with it. The shout woke up Tom who was now alert and a ready for the killers when they enter his room. A horse carriage had to make an emergency stop so as not to hit the old ladies. The horse carriage flips over and the commotion wakes up Tom who had fallen asleep. He is now awake and ready for his killers

| I think you might be on the right track but there was no shouting.

Was Tom asleep at first? Did something happen outside that caused him to wake him up? If yes, did it involve a horse carriage and driver in addition to the old ladies?

Did a horse from the carraige alert Tom? Maybe the horse whinnied because it had to stop for the old ladie, and when Tom looked out the window, he saw his killer and was able to prepare. Hmmm, I think that's wrong. Did Tom ever look out the window? Sorry if that's already been covered.

When I said that there were horse carriages, actually, there might have been horse carriages, but then again, not necessarily.

It was a stampede !! The old ladies all got crushed down by the horde of horses. Actually, I thought that Tom is in the room with the killers, going for a showdown. The old ladies crossed the street, oblivious to the procession of carriages coming down the street. The drivers tried to avoid the ladies, swerving and hitting the hotel wall, knocking it down. (Un)fortunately, the falling walls fall down onto the killers also. Guess Tom is lucky not to stand with his back to the wall.

Maybe the "few old ladies who were crossing the street" are in fact thousands of buffalo cows tearing down half of the town and incidentally running over the bad guy who was about to shoot Tom at noon.

Did the gunmen react to the old ladies crossing the street? (e.g. seeing them, hearing them, smelling them)
